I suspect I've listed some of these before, but came across them again recently, and wanted to make sure they weren't missed. (I really need to get these reports in a more easily searchable form.) All the Iowa reports come from this review article in the Des Moines Register.

Okay, an update. I kind of gave up on the idea of going one-by-one and inserting complete data into this spreadsheet:

and instead, wrote a little Python script to crudely extract the information from this hread. I then tweaked that output into more proper CSV form, cleaned it up a bit, then inserted it into the above spreadsheet. There are probably some duplicates. There is certainly plenty of data still missing, but it's a start.

I would really like some help cleaning this stuff up more, extracting the auxiliary data from the reference URLs, and summarizing what we've got here.
